Exciting opportunity to provide new models of care for walk-in/urgent care access including days, evenings and weekends. The new CHA CBHC Cambridge program provides behavioral health evaluation and initial treatment services for individuals of all ages. The Supervisor, Social Work provides day-to-day coordination and oversight of the staff in the CBHC, reporting to the Program Manager of the CBHC Urgent Care and the Director of Adult Outpatient Psychiatry. Runs team huddles and meetings as needed, performs triage, assigns cases and monitors caseloads, is the point person for clinical and administrative issues that arise on the team (such as Section 12), is responsible for clinical oversight of team patients, and provides handoff to the Program Manager on patient and staff issues. Acts as a conduit with the Mobile Crisis Intervention Team when in the supervisory role. The Supervisor, Social Worker functions as a clinical social worker in the service, providing evaluation and treatment services for patients.
Summary:
Responsible for the evaluation and treatment of an assigned caseload of clients, providing a range of psychosocial care. Works autonomously and provides administrative as well as clinical supervision to staff (social workers and others) and trainees (social workers and others). May provide programmatic administrative leadership, such as leading a team.
- May design and implement social work programming at one or more service sites.
- May participate on the management team of the relevant service site, or leadership team of the relevant service line.
- Provides psychosocial assessments, evaluations, and referrals for adults, children, and/or their families with psychiatric illness, substance abuse, and medical issues and illness. Casework may take place in the hospital, offices, or community settings, as appropriate.
- Provides a range of psychological and psychosocial treatment, including individual, couple, family, and group psychotherapy treatments and clinical case management services.
- Collaborates with multi-disciplinary team at service site, and specifically with the team leader to define and provide the services needed by clients. Demonstrates a capacity to work collaboratively in a team setting, with an ability to listen to others, participate actively, and take appropriate responsibilities.
- Collaborates with other caregivers and community agencies as necessary to provide systemically informed, community based treatment. Such collaboration may require participation in caregiver conferences, Patient at Risk meetings, and other meetings as needed.
- Completes billing forms, managed care documentation, statistics, and other required paperwork thoroughly and on time.
- Provides all necessary documentation in clients' medical charts in an effective and timely fashion.
- Participates in activities of the Cambridge Hospital Community Health Network Department of Social Work.
- Participates in teaching and training activities, including continuing education activities needed to maintain yearly licensing requirements.
- Provides clinical and administrative supervision for Clinical Social Workers and social work interns. May also provide clinical and administrative supervision for staff and trainees of other disciplines.
- May participate in the design and implementation of a program of social work training for social work interns and others in the Cambridge Health Alliance.
- May participate in PI activities, both in the service line/site and in the Department of Social Work.
- Facilitates access to entitlements and insurance benefits, and Department of Public Health and Department of Mental Health services. Assists patients/clients with applications and assists with follow-up as appropriate.
- May participate in the design and implementation of patient education and psycho-education programs for clients, families, and others.
- Performs other duties as necessary or as assigned by administrative supervisor, including providing evening and weekend social work coverage when necessary/assigned.
MINIMUM QUALIFICATIONS:
Masters degree in Social Work or an equivalent degree in a related field.
Education/Training: Masters degree in Social Work
Licensure: Current MA licensure as LICSW (Licensed Independent Clinical Social Worker) or eligibility for licensure.LCSW may be considered.
When required, must be credentialed by the CHA Medical Staff for practice in relevant clinical practice areas, such as individual, couple, family, or group psychotherapy.
Work Experience: Minimum of three years post masters degree, in clinical or human service setting with relevant populations
