Highland Village of Elko is seeking a motivated and enthusiastic Social Worker, LPN or RN to work with our amazing residents and coordinate the Social Service program in our retirement community focusing on admissions.

Job Duties Include:

• Develop good working relationships with facility personnel as well as outside social service agencies to ensure resident needs are met.
• Assist Admissions Director with tours, admissions and discharge planning
• Implement social care plans and resident assessments as needed
• Refer resident families to appropriate social service agencies if the facility doesn't provide the needed services
• Complete social histories
• Participate in care plans
• Document conditions and behaviors of residents to update care plans as needed
• Serve as liaison between facility, resident's family and outside agencies
• Provide social and psychological support to residents

Salary: DOE

Requirements:

• Bachelors Degree in Social Work or related field
• OR Bachelors Degree in Nursing
• 1 year experience in social service
• Background check
