General Statement of Duties: The Intake Coordinator, under general supervision, interviews and screens prospective clients for services; makes referrals to private and public service agencies as appropriate; conducts social assessment; gathers information and arranges diagnostic evaluations needed to determine eligibility; ensures compliance with statutory time requirements through intake; and develops initial client goals and development plans.
Working Condition and Physical Requirement:
- Duties are performed in the office and in the field. The ability to drive an automobile is required on a daily basis.
- The Intake Coordinator must maintain a valid drivers license and be able to travel throughout San Joaquin County.
- Travel throughout the five-county area will be required as workload demands.
- The job duties require the ability to sit or stand for long periods of time, and to operate office machines requiring manual dexterity and repetitive key entry.
- The ability to read, write and comprehend large amounts of written material, use the telephone, and communicate via electronic mail are required on a daily basis.
- Excellent oral and written skills are essential.
Key Responsibilities Essential Functions
1. Interview and screen perspective clients for services.
2. Monitor all clients through the intake process to ensure compliance with statutory regulations.
3. Gather information, attend related meetings, e.g., court, IEP, IFSP, and arrange for diagnostic evaluations for the determination of eligibility.
4. Conduct and write intake assessment. For ES IC, write intake as part of initial Individual Family Service Plan (IFSP).
5. Participate in ID Team meetings with other clinical staff. . Participate as part of assessment team for ES to determine eligibility, services and do initial IFSP.
6. Complete Client Development Evaluation Report (CDER) and administer measures of adaptive behavior. For ES, complete web based Early Start Report(ESR) .
7. Administer appropriate screening tools (M-CHAT-R or SCQ/SSSQ/K-BITetc) as needed
8. Provide resources to familys of consumers found not eligible for ES or ongoing VMRC services (crisis intervention services).
9. Provide information and/or counseling to consumers/families regarding the identification and development of consumer goals and development plans.
10. Make consumer referrals to private and public agencies as appropriate.
11. Coordinate the Eligibility Team Staffing.
12. Identify goals and objectives for the Individual Program Plan (IPP) or IFSP
13. Maintain adequate skill level in interpersonal relations.
14. Prepare reports as requested, meeting agency timelines and quality standards.
15. Coordinate due process cases. Attend due process meetings.
16. Attend agency and community meetings as requested.
17. Other job related duties as assigned.
Minimum Position Requirements: Education and Experience: Education and Experience: Masters degree in Social Services or related field and one year of experience working with the developmentally disabled or Bachelors degree in Social Service or related field and three years work experience with developmentally disabled. Preference will be given to applicants with VMRC experience and to those who have bilingual skills.
Knowledge and Abilities: Knowledge of both developmental disabilities and normal child developmental milestones, service needs of the developmentally disabled and developmental disabilities statutes. Ability to interact sensitively and positively with potential clients, their families, other VMRC professional staff and community professionals.
Compensation: $27.41 per hour starting
