Position summary:

The Housing Support Specialist is responsible for assisting the clients of the Good Shepherd Center to obtain sustainable housing. The Housing Support Specialist will help clients navigate Rapid Rehousing and Permeant Supportive Housing Programs, complete needed assessments, maintain client records, connects clients to education and vocational training as well as career/job counseling and development, maintain client, resident, guest and organizational confidentiality in compliance with organizational policies and procedures. The scope of the program will include helping guests complete a housing programs and apartment applications, helping prepare guests for interviews with landlords and apartment managers, and advocating for a guest in order to obtain housing.

Qualifications

  • Bachelor's (Preferred)
  • Bilingual (Preferred)
  • Microsoft Office: 1 year (Preferred)
  • Case Management: 3 years (Preferred)
  • Customer Service: 3 years (Required)
  • Driver's License
  • Be familiar with Rapid Rehousing, Section 8, and Supportive Housing Programs
  • Be familiar with ADA and Fair Housing Regulations
  • Strong Interpersonal Skills
  • Strong conflict resolution skills
  • Basic IT skills
  • CPR/ First Aid certification or the ability to get one within 90 days of start date.
  • Ability to lift up to 50 lbs.
  • Ethics training on HIPAA guidelines

Position Duties and Responsibilities

  • Provide housing related case management to homeless clients who are guest of the Good Shepherd Center.
  • Services include: outreach and engagement, screening, intake and assessment, document acquisition, service plan development, linkages and referrals to clinical and appropriate community resources, housing search, leasing and moving into housing.
  • Assist clients in acquiring skills and resources necessary for successful community living
  • Create and Implement the Individual Responsibility Plan.
  • Meet with assigned residents individually to review progress toward goals and provide case management and skill building interventions to improve vocational skills and ensure housing stability.
  • Comprehensive coordination of care including developing/maintaining relationships with community resources for successful transition to and maintenance of independent living.
  • Complete additional tasks as requested by Clinical Director or Program Manager.
  • Develop individualized housing plans for guests.
  • Provide (support to guests in finding and maintaining housing by contributing to the enhancement of) skill building activities for daily living
  • Refer guests to other resources if the Housing and Reestablishment Program does not meet their needs.
  • Provide supportive direction and skills development in order to assist guests in preparing for interviews with landlords and employers.
  • Follows Health Insurance Portability and Accountability (HIPAA) guidelines, as well as have received ethics training
  • Participate in regular staff meetings, and attend internal or external meetings as directed.
  • Perform any other related tasks that may from time to time be assigned by the Center Executive Director, or delegate.
  • Manage the admissions process for individuals receiving supports and services
  • Create and maintain service authorizations
  • Gather necessary documents for all new admissions
  • Participate in pre-placement meetings for all new admissions
  • Ensure all necessary documents are collected prior to admission
  • Ensure new individuals and amendments to services are entered in Case Management System

Health and Safety

  • To observe all health and safety rules and regulations and work in manner that minimizes risk to yourself, clients, co-workers, as well as others and the property of the Good Shepherd Center. To exercise managerial responsibilities in accordance with the Occupational Health and Safety Policy and Occupational Safety and Health laws, rules, and regulations.
  • To ensure that shelter staff and volunteers work in the manner and with the protective devices, measures and procedures required by Occupational Safety and Health policies, rules, and regulations.
  • Comply with established rules of operation, procedures, policies, and code of conduct.
  • Comply with work scheduling and attendance requirements according to policy and practices.
  • Collaborate with program managers/departments to improve communication and cohesive service delivery throughout the Good Shepherd Center.
  • Participate in the implementation of all required Standards as directed, i.e. Shelter Standards, Medical Officers Standards, Health and Safety Standards, Privacy Standards and policy requirements.
  • Respect the confidentiality of service user information, as well as other information that is sensitive in nature to the operation of the Good Shepherd Center, staff, volunteers and others.
  • Maintain confidentiality and ethical standards at all times.

Create a Housing Sustainability Plan that is focused on housing stability with each program participant including:

  • Budgeting
  • Life Skills Training
  • Direct/Assistance in applying/securing all eligible entitlements
  • Reducing other barriers to housing
  • Conduct interim assessments and submit forms as needed
  • Monitor client progress through home visits, field visits, office visits and telephone contact
  • Provide crisis intervention as necessary
  • Assist in building landlord/tenant relationship
