We are seeking an HOME CARE PROVIDER to join our team!

We are looking for an experienced Home Care Provider to support our client with a 2:1 ratio requirement. This role would require our incoming home care provider to have an experienced parter to work with OR a willingness to work alonside another competent and experienced homecare provider, in their home, yours or another property. This client requires 24/7 support from their home care provider and has extreme behavioral needs.

The HCPs for this client will be provided 30 hours of direct support staff each week for assistance.

Responsibilities:

• Manage severe behavior needs
• Administer nursing care to ill, injured, or disabled patients
• Help establish patient treatment plans
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• Educate patients about health maintenance and disease prevention
• Facilitate referrals to other healthcare professionals and medical facilities
• Maintain accurate patient medical records
• Provide advice and emotional support to patients and their family members

Qualifications:

• Previous experience will extreme behaviors is a must
• Maintain a calm demeanor in the face of physical and behavioral aggression
• Ability to build rapport with patients and families
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment
• Commitment to skill development, independent living, and full community participation for all individuals.
• Current medication administration and CPR/First Aid Certification required (will train).
• Completion of CPI behavior training (will train)
• A valid NH driver's license.
• Auto Insurance at or above NH state minimum.
• Homeowners or renters insurance.
• Must pass a National and Local criminal background check, BEAS, DMV records check, and required TB testing.
