Position: Health Information Management Coordinator

Background:
Maimonides Midwood Community Hospital is a 134-bed, non-profit acute care hospital located in the
Midwood and Madison Park section of Brooklyn. In clinical partnership with Maimonides Medical
Center, our hospital has a dedicated team of over 150 physicians, among them Board Certified
physicians in 31 specialties, including Internal medicine, Cardiology, Gastroenterology,
Pulmonology, Infectious Diseases, Vascular Surgery, Gynecology, Plastic Surgery, Nephrology,
Pain Management, Ophthalmology, Oncology, and Surgery, and subspecialties in Vascular, Cardio
Thoracic, Colorectal Surgery and Urology. We provide the highest level of care for patients in the
communities we serve.
Role
The Health Information Management Coordinator works to facilitate the overall quality and
completeness of clinical documentation to accurately represent the severity, acuity and risk of
mortality profile of the patient. Focused communication with the treating clinical professionals (i.e.
queries) will be utilized to obtain improvements in documentation. Additionally, the Health
Information Management Coordinator is also responsible for chart analysis, maintaining and tracking
clinician incomplete charts and assisting with transcription processing, coding workflow processing
and ROI.
Health Information Management Coordinator Responsibilities:
 Assists in the development of processes and initiatives designed to improve clinical
documentation performance
 Demonstrates effective communication skills and collaborates with medical staff, clinical
departments and key facility leadership team members
 Assists CDS with the medical staff regarding CDI activities including following up on securing
responses to the physician queries.
 Coordinates with HIM Coding staff retrospective queries.
 Maintains and demonstrates knowledge of CDI/HIM processes and best practices
 Coordinates and holds CDI team to key performance indicators
 Maintain/monitors query/query logs
 Presentation and report creation and editing skills.
 Effective written and verbal communication skills with staff and stakeholders.
 Assess, coach, train, and develop talent.
 Enters, edits, and tracks medical record deficiencies in chart management system.
Effectively communicates with physicians to resolve and manage physician deficiencies.

 Analyzes records in accordance with the HIM analysis procedure with a minimum of 95%
accuracy.
 Performs quantity and quality checks on scanned medical record information as part of
Quality Control function.
 Compiles, updates and reports physician record completion statistics and physician
notification letters. Prepares physician list and handles the suspension process.
 Produces and distributes delinquency notification/suspension letters, faxes and emails.
Verifies accuracy of deficiency information, reports deficiencies to medical staff leadership.
 Serves as liaison to physicians regarding issues related to incomplete records and
documentation to ensure adherence to Joint Commission standards and compliance
requirements.
 Facilitates chart availability and readiness for coding.
 Release of Information of Legal requests. Accepts subpoenas and coordinates with legal on
all legal requests.
 Continuously meets all turnaround times as requested.
 Monitors transcription system.
 Continuously meets all quality/quantity requirements.
 Assists visitors and physicians, and answers phones as needed.
 Responsible for cross training peers.
 Assists the HIM Department and various work units during times of staff shortage or high
volumes.
 Compiles statistical data.
 Performs other tasks as assigned
Education and Experience Requirements:
 Minimum 2 years of experience working with healthcare records or a hospital HIM
department required
 Associates degree in HIM preferred.
 Proficiency in ICD-10 terminology

Job Type: Full-time

Pay: $45,000.00 - $55,000.00 per year

Schedule:

  • 8 hour shift
  • Day shift
  • Monday to Friday
