Riverside Home Health and Hospice is a Medicare-certified agency that is seeking two experienced and caring full-time Hospice (1) and Home Health (1) Clinical Managers for our Home Health and Hospice divisions in Grants Pass, Oregon.

Qualifications

• Oregon Licensed RN (RN)
• Ability to work independently and demonstrate critical thinking
• Current BLS certification from AHA
• Be a licensed driver with an insured automobile in good working order

Essential Duties

• Provide one-on-one skilled nursing care to patients in their homes (e.g., wound care, catheter care, medication administration, symptom management, pain management, etc.)
• Assist Director of Clinical Services with planning, implementation, and evaluation of CEU programs
• Assists with training, mentoring, and supervising agency personnel
• Assist in screening, interviewing, & making recommendations for new personnel
• Review referrals, care plans, documentation, orders, etc. for appropriateness and compliance
• Participates in QAPI, PR and agency and community activities promoting agency roles and goals
