The Associate Director of Population Health will work directly with our VP of PQN, Chief Medical Officer,
Operations Team and Population Health Leadership to ensure the success of our ACO in the Delaware
market. They will be responsible for a wide-variety of areas, including managing value-based contracts and
relationships, physician engagement, new population health initiatives, and operational oversight as the
Delaware Population Health Team grows.
Primary Job Duties:
- Key stakeholder: the primary point of accountability for delivering success in Delaware Value based contracts
- Works collaboratively with Market operations team and National Value Based Care team to
- Analyzes population health and utilization metrics to determine how best to deploy clinical staff in partnership with Physician leadership and with support from medical economics teams
- Leads regular physician meetings, both group and one-on-one, to review population health
- Trains specific physician practices on how to transform their group and staff to the new world of value-based care, on topics including Quality, Risk Adjustment, and Total Cost of Care
- Meet with key payer stakeholders and contracts in the market
- Oversees programs to ensure providers are meeting MSSP program requirements and others as they are added
- Willing to complete chart chase, attestations and other tasks integral to success as necessary
Qualifications
Bachelor’s degree preferred. Clinical degree, Healthcare Master’s or MBA preferred.- At least 7 years of experience working in health plans, health systems, or provider organizations preferred
- Expert in value-based payer programs, risk, and successful tactics to improve quality and generate
- Ability to interact with all levels including senior management and influence decision-making
- Able to grasp both clinical and business concepts
- Analytical, quantitative, and metrics driven; must know how to interpret value-based program
- Project manager able to juggle multiple projects and urgent deliverables across a wide variety of
- Demonstrated ability to work with, train, and collaborate with clinicians
- Must comply with HIPAA rules and regulations
