United Community Options serves children and adults with a wide range of needs and abilities including physical, intellectual, and neurological disabilities, as well as developmental delays and developmental disabilities, such as cerebral palsy, Down syndrome, autism spectrum disorder, Prader-Willi syndrome, epilepsy, traumatic brain injury (TBI), and those who are technology dependent or medically fragile. In addition, we serve typically developing children and adolescents in many of our programs.

  • Assess needed areas for training and acquisition for independent living (i.e., money management, emergency procedures, etc.)
  • Assist/train adults with ADL, to include personal care, clothing household chores, laundry, and shopping and meal preparation.
  • Complete all required paperwork, including monthly progress notes med charts, behavioral data benefits application, medical summary, etc.
  • Attend weekly meetings with Program Coordinator and all other Agency wide meetings (i.e., team behavioral meetings, IPP’s, ISC’s
  • Meet regularly with each adult, and on an as needed basis with family, physicians, Support Coordinators, therapists, etc.
  • Assure overall maintenance and cleanliness of each facility and maintain adequate supplies and equipment therein.
  • Maintain banking ledgers, petty cash logs and assist with individual budgetary needs.
  • Order medications and lab work and assist with all medical, psychiatric appointments and follow-up as needed.
  • Oversee and assure that all emergency oriented issues are addressed, including training, equipment, phone numbers, manuals, etc.
  • Encourage and assist facilitation of adults with the development of outside activities (clubs, religious organizations, classes, etc.)
  • Ensure all transportation needs are met (setup B&L, drive van, train individual in the use of Public and Private transportation). Utilizing own vehicle as needed..
  • Assure replenishment of all supplies (food, medication, cleaning products, etc.).
  • Attend in-services, seminars, workshops, job sites, educational facilities and meet all other agency requirements.
  • Follow agency policy and procedure and designated work schedule (include On-call, etc.).
  • Ensure that program’s promote the highest level of independence in each resident.

Job Type: Part-time

Pay: $20.00 per hour
