SOR Correction Liaison-Case Manager- We are looking for a dynamic individual to join our multidisciplinary behavioral health team. Applicant must be interested in the recovery process and be eligible to obtain a CADC (Certified Alcohol and Drug Counselor). Hours will primarily be Monday through Friday 9-5:30 but will require flexibility based on client need as well as community outreach.
Job Summary: The basic tasks are to coordinate the case management needs of individuals with a history of Opioid Use Disorder that will be or recently have been released from a correctional setting. These individuals have been identified as being in need of case management services due to their substance use and criminogenic needs. The employee in this position will be a detail-oriented team contributor with strong enthusiasm and a passion for learning, who is able to excel with minimal supervision. In addition to possessing excellent verbal and written communication skills, the employee should have excellent listening comprehension, including the ability to understand complex written and verbal instruction. Because of the nature of the information with which the employee will be working, this position does require a high degree of confidentiality. This employee will work directly under the Outpatient Addiction Program Supervisor in the Northern service area. Clearly understanding the agency mission, goals and direction as set by the Board of Directors is essential. Developing a clear understanding of the agency philosophy and the directions of the supervisor and supporting his/her direction is required. The employee must function well in a political environment where critical management decisions are made on an ongoing basis and able to interact in this intense environment in a courteous and professional manner with tact and poise. The employee will need to perform a variety of important, sometimes urgent, confidential and complex tasks with constantly changing priorities. Education, Certifications, & Experience -
- Bachelor’s degree from an accredited educational institution, preferably in the social or behavioral sciences, AND
- Certification as a Certified Alcohol and Drug Counselor (CADC) or International Alcohol and Drug Counselor (IADC) from the Iowa Board of Certification, AND
- Two or more years of experience providing direct outreach and service delivery with underserved and/or hard to reach populations with substance use or co-occurring conditions.
- Ability to communicate effectively, regularly, and fluently in English; bilingual (English/Spanish) abilities are preferred but not required.
- Ability to read and interpret documents.
- Ability to complete written reports and correspondence in a timely and accurate manner as needed
- The staff member must have the ability to move within and between agency buildings and other locations where program services are provided – travel required.
- The ability to maintain the confidentiality of client information as mandated by various federal and state laws.
- The staff member may be required to work at any agency building as needed/directed and be responsible for her/his own transportation.
- The staff member will be scheduled based on agency need.
- The ability to understand all policies/procedures and the ability to reference them from the agency’s manuals.
- The staff member must be able to work in a fast-paced environment with demonstrated ability to juggle and prioritize multiple, competing tasks and demands and to seek supervisory assistance as appropriate.

Primary duties and responsibilities of the position -
- Provide service coordination (case management) for an assigned group of clients, including coordinating and monitoring the activities of the individual treatment team.
- Engage prospective clients prior to release and after re-entry to assist with connecting to Medication Assisted Treatment (MAT) services, counseling, and recovery support groups.
- Actively collaborate with local and regional corrections staff in order to coordinate services for individuals with opioid use disorder prior to release or after re-entry.
- Provide individual supportive case management and symptom management.
- Complete, review, and participate in client-centered comprehensive evaluation and assessment for individuals awaiting release when needed.
- Document client progress to maintain a record of client activity according to established methods and procedures.
- Participate in staff organizational meetings and treatment plan review meetings.
- Provide advocacy, linkage, and outreach services for clients and their families.
- Provide transportation for client appointments as needed.
- Provide accurate data and coordinate with data outcome reporting including completing GPRA intake, discharge and follow ups in a timely manner.
- Attend and participate in community stakeholder group meetings to identify solutions to barriers that individuals who are incarcerated may encounter as it relates to re-entry and substance use disorders.
- Advocate for clients’ rights and preferences.
- Other duties may be assigned.
Job Knowledge -
- Knowledge in the theories, principles and techniques of substance use treatment and criminogenic treatment.
- Must have a good understanding of criminogenic risk factors and their interaction with both mental health and substance use disorders.
- Knowledge of opioid overdose and response, the basics of Medication Assisted Treatment (MAT), harm reduction, and health equity.
- Knowledge of 42CFR and HIPAA as well as how it applies to Community & Family Resources
- Knowledge of the psychosocial aspects of substance use,
- Knowledge of the principles of recording and report writing,
- Knowledge of CARF and IDPH licensure standards, and
- Knowledge of public/private welfare, health care, problem gambling and substance use agencies available in the area served by this agency.
- Ability to establish good working relationship with clients and staff.
- Ability to secure appropriate information by direct and collateral interviews.
- Ability to analyze problems presented, to bring agency/community resources to bear within policies and regulations, and to develop and implement effective courses of action.
- Ability to manage multiple tasks at the same time.
- Ability to diffuse and manage stressful events personally and professionally.
- Ability to develop, implement, and evaluate Quality Assurance practices within the program.
- Must have a strong commitment to the right and ability of each person to live in normal community residences; be part of the workforce; and have access to helpful, adequate, competent, and continuous supports and services.
- Ability to establish supportive trusting relationships with persons within the criminal justice system.
- Must demonstrate respect for client rights and personal preferences in treatment.
- Must have a valid driver’s license for the state in which the program operates and have reliable transportation.
