Regional West Health Services is actively looking for individuals to join the team and a Social Worker/Medical Social Services Representative.

To serve this mission, you will provide discharge planning and psychosocial interventions through assessment, advocacy, referral and interdisciplinary planning for identified patients.
As an RWHS Social Worker/Medical Social Services Representative, you will:
- Provide objective casework services to patients and their families which include: social assessments, problem identification, evaluation, planning, referral, education and counseling related to social, financial and economic issues
- Educate patients and families regarding resource options
- Communicate with physicians, ancillary staff and serve as a resource to the healthcare team
You may be a good fit if you have:
- A degree in Social Work from a program accredited by the Council on Social Work Education OR a Bachelor’s Degree in a human service related area
- Certification as a Social Worker in the State of Nebraska, desired but not required
- Two years of experience in a human service area
