Assistant Social Services Responsibilities:
Develop Transfer notices, room change notifications and issue cut letters
Participates in the update of insurance
Develops Medicaid applications and assisting with financial side of stay and knowing the up-to-date insurance benefits and private pay
Develops, coordinates, and participates in long term care conferences and short-term care conferences once a month
Develop discharge planning and home health referrals
Documents levels of cars/9401s
Job Requirements:
High School Diploma
