We are seeking experienced, reliable and highly motivated Assessment Nurses to conduct in home assessments for prospective and current members to determine initial plan eligibility, continued eligibility and their health care needs using the UAS tool.
This position requires a current New York State Nursing license and two years minimum experience in home care or care management. The ideal candidate must possess excellent listening and communication skills, the ability to complete documentation in a timely manner, as well as attention to detail and accuracy when performing assessments. Must have access to reliable transportation. Bilingual in Mandarin/Cantonese required-fluent in Korean a plus.
