Exciting opportunity for an RN Care Coordinator in our Roaring Fork Family Practice!

As the RN Care Coordinator, you will work collaboratively with providers and clinical staff to support patients with chronic conditions and /or complex needs according to guidelines established by the primary care provider and in conjunction with patient established goals.

QUALIFICATIONS:

Requirements:

Education:

• Current Colorado RN license or ability to obtain
• Current BLS through American Heart Association

Skills:

• Effective communication – both verbal and written
• Excellent interpersonal skills
• Ability to affect change
• Ability to perform critical analysis
• Self-Directed
• Positive Attitude

Experience:

• Minimum 2 years RN work experience.
• Three to four years RN experience in the area of specialty, is preferred.
• Experience in Care Management a plus

SALARY RANGE

• ‎$31.43 to $46.21/hour
• Entry salary dependent upon education, skill set, and experience.
