MPNN CSB is seeking highly qualified professionals to be on the clinical team for the newly formed Program of Assertive Community Treatment (PACT). Employee to provide psychiatric nursing support, and wellness initiatives for PACT. The employee ensures the provision of the PACT Model, Assertive Community Treatment and hospital diversion services to adult SMI/COD consumers in the Middle Peninsula Northern Neck Community Services Board (MPNN CSB) area via program coordination, direct service, consultation and education, as well as other intra and interagency functions. Works with a challenging population, often from diverse backgrounds with a variety of concomitant issues. Must be able to work a flexible schedule, including some evenings, weekends, and holidays. Provides services in the community as well as in client homes. Supports the implementation and maintenance of electronic medical records and data collection. This positions incumbent serves as a Nurse/Case Manager on an interdisciplinary team (PACT). This position works closely with other staff at MPNN CSB using a team approach. VA licensed LPN or RN with considerable experience in a hospital, clinic, or community mental health setting with significant experience working with the mental health population and psychotropic medications. Must possess a valid Virginia Drivers License. Must obtain a TB test with satisfactory results by orientation and annually thereafter as required by licensing authority.
