RN Case Manager! Monday-Friday (days) and a
$10K Sign on Bonus!
You will report directly to the Executive Director or Patient Care Manager Senior and will maintain primary control and professional management of each patient. You will act as primary liaison between physician, patient/family and hospice team. Our hospice team is dedicated to providing compassionate care and extraordinary service to the patients and families we serve.
What to Expect:
- Flexibility on scheduling patients throughout the day
- One-on-One Patient Care in their homes or facilities
- Some rotating on call coverage required (extra pay!)
- Mileage reimbursement
About You:
- Compassionate, kind and empathetic to your patients and their families
- A current RN license (in the state of requested employment or ability to obtain)
- 1+ years of nursing experience in hospice, home health, or oncology preferred
This role requires full COVID-19 vaccination.
