Position Responsibilities: Participates in developing education and prevention programs and coordinating health programs for the total well-being of sisters.

· Health and safety assessments (physical, social, psychological, spiritual and environmental) so that the sister, her local community, the Health Care team and council delegate have an awareness of the individual’s situation.

· Education and process regarding assessments and ways they will be utilized.

· Provision for some kind of “safety net,” especially for sisters who are more isolated or in remote locations, so that their health care needs do not go unnoticed.

· Follow protocols to guide transitions from hospitals to home, rehab, Sinsinawa Mound, etc., considering placement and duration, etc. in dialogue with appropriate parties.

· Follow protocols to guide communication with a sister’s Council delegate regarding major changes in a sister’s health status and aging issues. Together they will talk about moving through various life stages.

· Provision of various health and wellness education programs promoting

prevention and holistic healthy self-care.

· Assisting sisters in finding and utilizing local community resources as needed; facilitating the acquisition of services that allow a sister to maintain independence.

· Assisting sisters who need temporary services at a rehab or skilled care to

obtain appropriate placement.

· Assuring that sisters who are hospitalized have assistance with discharge planning and after-care.

· Serving as a sister’s advocate when appropriate.

· Collaboration, coordination, and communication with Sinsinawa Mound and Villa staff especially when a sister is being transferred to one of these locations.

· Perform other duties as assigned by supervisor.

Job Types: Full-time, Travel nursing

Pay: From $76,500.00 per year
