Registered Nurse Job at Greater Portland Health
ESSENTIAL JOB FUNCTIONS: *
- Perform initial patient assessment, including review of hospital charts, discharge paperwork, and collateral information.
- Explain discharge paperwork to patients and create and help patients execute plan for medical compliance.
- Monitor and provide daily insight on RCP bed occupancy while working to ensure patient compliance.
- Implement physicians’ orders for interim housing patients, administer medications including intravenous medications, perform treatments, procedures and special tests, and document treatment for our patients as required by Greater Portland Health policy and local/state rules and regulations.
- Provide wound care and PICC care as indicated.
- Assist patients with medication management practices.
- Coordinate with on-site team to write medical care plans for each patient and serve as liaison with hospital staff.
- Update patient charts in electronic medical record upon admittance to the RCP, during stay, and at discharge from program.
- Provide basic medical care and education for patients.
- Apply sound nursing judgment in patient care management decisions.
- Administer over the counter and prescription medications as ordered.
- Screen for advance directives; set-up advance directives for patients who do not already have them.
- Connect patients to a PCP per patient choice.
- Engage patients in utilizing medical home to avoid inappropriate emergency room use.
- Coordinate different types of health screenings and follow-up care as needed.
- Keep private and secured records of health-related procedures and activities.
- Participate on related committees.
- Perform other duties as assigned.
