RN - Registered Nurse for Admissions for a LTC facility on Eastern Long Island -working for a well known facility in Southampton , NY. Must have Admissions Experience and be flexible. Earn $52 to $53 Per Hour depending on shift.
Admissions Experience a must
- RN Licensure New York State
- BLS
- Flex Shifts; Must have availability every other weekend
- Perform head to toe assessment of new residents.
- Have the ability to assess, plan, implement and evaluate nursing care according to the individualized needs.
- Communicate and collaborate with the interdisciplinary team and or physicians.
- Complete all paperwork for the Nursing Admission Assessment.
- Enter all data into Electronic Medical Record.
- Obtain orders from MD and ensures accuracy of Admission Orders in EMR.
- Completes the Admission Nursing Note.
- Complete the Fall Risk Assessment and intervention care plans as indicated (sensors, alarms, etc).
- Establish and maintain communication processes with all patient care.
- Comply with all state requirements and facility policies, procedures and standards of conduct.
- Completes the Pressure Ulcer Risk Assessment / Braden Scale. If high risk ensures all preventative measures are implemented.
- Obtain consents: Pneumovax / Flu and places in the patients’ chart.
- Ensure Initial/Interim Care plans are put in place. Fall Risk, Skin Alt-Potential, Pain-specific resident conditions (pressure ulcer, post-op, infection, etc).
