The Referral Specialist works under the direction of the Clinic Manager as part of a multidisciplinary team composed of physicians, nurse practitioners, physician assistants, and clinical support staff. CommuniCare Health Centers is a Federally Qualified Health Center providing comprehensive patient-centered primary care services including medical, dental, substance abuse treatment, behavioral health and outreach services to the low income, multi-ethnic, uninsured and underinsured in the communities we serve.


SCHEDULE: This is on-call position. 0-40 hours per week, Monday Friday at Salud Clinic in W. Sacramento. Travel between clinic sites may be required.

RESPONSIBILITIES
:

  • Coordinates the processing of patient referrals at designated site including correspondence to referral agencies, providers, authorization services, and patients. Follows the prescribed referral process provided during the orientation training period.
  • Follows up on all calls and correspondence from outside specialists in a timely and professional manner.
  • Provides regular updates to providers as to the status of referral scheduling requests.
  • Ensures that protocols are used to complete specialist follow-up requests for patient referrals.
  • Monitors patient charts to ensure the timely placement of all correspondence in the chart.
  • Works closely with the Clinic Manager and Director of Clinic Operations to coordinate logistics in terms of patient transportation, on-time arrivals to specialists, and patient follow-up.
  • Assist with other projects as assigned.

MINIMUM POSITION REQUIREMENTS:


Education/Experience
: College coursework or one year experience in a medical office, hospital or clinic. MA training a plus. Administrative experience in a health care setting. Experience working in a health clinic and demonstrated commitment to low income populations a plus.

Skills and Abilities:

Computer skills to include Microsoft Office (Word, Excel, Outlook). Knowledge of medical terminology and ICD-9 codes preferred. Attendance and punctuality is necessary. Ability to work well with a multidisciplinary team. Ability to problem solve and be self-motivated. Must have a commitment to excellence and high standards. Must have effective and flexible written and oral skills, strong organizational, problem-solving and analytical skills. Must have ability to manage priorities and workflow. Required to have versatility, flexibility and a willingness to work within constantly changing priorities with enthusiasm and have acute attention to detail. Expected to have strong interpersonal skills and have the ability to understand and follow written and verbal instructions. Ability to work independently with little or no supervision. Required to have the ability to deal effectively with a diversity of individuals at all organizational levels and with external customers.

Ability to operate office equipment, including computer equipment. Must be able to sit or stand for long periods of time, and walk regularly. The employee must occasionally bend, lift, carry and/or move up to 25 pounds. The employee is occasionally required to bend, kneel, life, twist, reach with hands and arms, and to lift 25 lbs. Hearing required. COMPLETED COVID-19 VACCINAITON REQUIRED

PAY RATE

  • The pay range for this position is $18.20/hr.-$24.70/hr. Starting pay within this range will vary depending on the job-related knowledge, skills, and experience.
  • May be eligible for annual 2% + 0-2% merit-based pay adjustment (0-4%).
