Quality Coordinator
Centerpoint Health is an Federally Qualified Health Center in Middletown, Franklin, and Norwood Ohio and has an opening for a part-time Quality Coordinator. This position is scheduled for 24 hours a week.
Retention Bonus paid at six months and twelve months totaling $1,000!!!
Description of Position/Purpose:
Identify, coordinate, and evaluate the collection and management of clinical data, assuring compliance with regulatory agencies and registry guidelines.
Job Duties:
· Assists in the coordination and implementation of the health center quality improvement/quality assurance (QI/QA) program, including the analysis of and efforts to improve performance to support the provision of high-quality patient care while maintaining patient confidentiality.
· Oversee tasks such as identify improvement opportunities and goals, implement action plans and activities, monitor/assess data, evaluate performance measures and outcomes, and update procedures. Specific focus areas include but are not limited to clinical guidelines and standards of care, patient satisfaction and grievances, patient safety and adverse events, and risk management areas such as tracking and managing referrals, diagnostics, and hospital admissions.
· Tracks and monitors data, identifies trends, and reports practice efforts regarding meeting internal and external clinical/quality of care measures and assessments. Assists with corrective actions and improvements as appropriate in order to meet regulatory/accrediting agency requirements with the goal of better patient safety and quality of care outcomes.
· Assists in communicating and educating and scheduling patients, families, and clinic staff, including physicians, about quality improvement projects, requirements, and efforts.
· Assists in development and monitoring of Patient-Centered Medical Home certification requirements and related Health Center activities, including clinical best practices and administrative workflow processes such as patient registries, self-management goals, hospital and referral tracking, etc.
· Works collaboratively with leadership and support staff and assists with compliance, regulatory and accreditation surveys and site visits, including but not limited to the Health Resources and Services Administration (HRSA).
· Other duties as assigned.
Credentials:
Education and Licensure:
Bachelor’s degree in health-related field or nursing (licensed with the state of Ohio), preferred.
Experience:
Experience in Quality improvement, preferred. Healthcare experience helpful.
Additional Skills/Qualifications:
· Possess excellent analytical and critical thinking skills.
· Be computer proficient with experience in the Microsoft Office suite or programs and electronic health records.
· Have strong communication, planning, and time management skills.
· Experience with healthcare quality improvement and outcome measurement processes
Bloodborne Pathogens Exposure: Category II
