Broadstep Behavioral Health is seeking a qualified professional to join our community based team working with adults suffering with mental health.
Job Duties:
- One on one visits in the member's home
- Participation in daily team meeting
- Documentation on client visits
- Service planning and authorization process
- Coordination of care with MCO's
- Linking clients to community resources
- BA/BS in Human Service
- At least 2 years of experience
- Previous experience with writing PCPs
- Must be flexible
