Psychiatric Mental Health Nurse Practitioner Job at Katahdin Valley Health Center
POSITION SUMMARY
The Psychiatric Mental Health Nurse Practitioner will work with KVHC patients in providing outpatient community mental health services. The Psychiatric Mental Health Nurse Practitioner will collaborate with the Primary Care Providers to recognize, treat and manage mental health and psychosocial issues while acting as a contributing member to the Integrated Primary Care Team.
RESPONSIBILITIES
1. Provides focused intervention for patients who are in need of mental health services
2. Obtain a Mental Health history and diagnostic evaluation
3. Interpret health data by identifying wellness and risk factors
4. Evaluate, diagnose, intervene, and monitor neuropsychiatric problems.
5. Identify variations from norms
6. Prescribe medication per the approved formulary based on licensure with the Maine Board of Nursing and current DEA registration.
7. Monitor patients' medication usage and results.
8. Educate patients and family members about mental health and medical conditions, preventive health measures, medications, or treatment plans.
9. Counseling individuals and families.
10. Consult and/or collaborate with other health care providers and community resources to influence care of patients and affect change in systems.
11. Initiates follow up to determine any changes in treatment approach and refer patients to other health care providers as appropriate.
12. Identify and provide for the education needs of the patient’s family members, considering age and culture.
13. Provide case management skills to coordinate comprehensive health services and ensure continuity of care.
14. Evaluate client progress in attaining expected outcomes.
15. Provides complete documentation of visit and the patients progress in the medical record
16. Maintain up to date records and billing practices in a timely manner within KVHC’s protocols
17. Works where indicated to effect behavioral changes in patients with or at risk for physical disorders
18. Assist patients in need of developing daily and community living skills
19. Provides patients with self-management skills and education information needed so they can participate in their treatment
20. Provides consultation with KVHC Management and other clinical team members about behavioral services and suggested areas of program evaluation
21. Assists patients with complying with medical treatment such as offering strategies to cope with medication side effects or chronic pain
22. Work closely with psychiatrists, psychologists, clinical social workers, psychiatric nurses and other mental health professionals
23. Reports as required to the QI/QA Committee
24. Attends required meetings and participates in committees as required.
25. Fully maintains professional decorum at all times.
26. Performs tasks as required by the Medical Director or supervising physician to promote proper utilization of time and staff.
27. Maintains accurate and complete patient health records to assure confidentiality and compliance with health care regulations.
28. Performs additional duties or projects as assigned by Medical Director.
29. Adheres to all KVHC policies and protocols.
30. Complies with all safety rules and protocols. Reports all incidents, regardless of severity, immediately to supervisor and completes an incident reports and investigation form, which will be forwarded to Human Resources within 24 hours of the incident.
31. Be a good steward in the communities serviced by KVHC; represent the organization at outreach activities as needed.
EDUCATION AND EXPERIENCE
1. Possess Bachelor’s Degree in Nursing followed by a Master’s Degree in Psychiatric or Mental Health Nursing from an accredited school.
2. Licensed as a Psychiatric Mental Health Nurse Practitioner in the State of Maine
3. Possess Psychiatric Nurse Practitioner certification from the American Nurses Credentialing Center (ANCC)
4. Current CPR and ACLS Certification.
5. Must possess the ability to be credentialed and covered by FTCA Malpractice Coverage
6. Valid State of Maine Driver’s license and ability to be insured by KVHC’s vehicle insurance carrier.
7. Must be possess a clean criminal and DHHS background.
8. Must be possess the ability and means to travel between clinics as needed.
