We are hiring Program Manager for one of our clients in Trenton, NJ.
Job Description:
- The Department of Children and Families is a partner on the Garrett Lee Smith Grant to help prevent suicide attempts and deaths by suicide in New Jersey among youth 10 to 24-year-old, as well as reduce subsequent suicide attempts and hospitalization.
- This vacancy is for a Regional Care Coordinator-Clinician whose responsibilities include, but are not limited to:
- Support the implementation of the Regional Care Coordinator project
- Provide clinical oversite with minimal supervision
- Partner with hospital staff to obtain consent and contact information for eligible youth
- Provide follow-up phone contact to youth and family after discharge from the emergency room and behavioural health facilities after a suicide attempt to engage the youth and family and connect them to appropriate services
- Data entry, maintenance, analysis, and reporting in compliance with HIPAA
- Data entry and tracking information of youth from different hospitals and facilities across the state.
Skills Required:
- License: Licensed mental health provider, i.e.: Licensed Clinical Social Worker, Licensed Professional Counsellor, Licensed Psychologist.
Experience Required:
- Three years’ experience working with youth who have experienced suicidal ideations, behaviors, and attempts.
- Experience with crisis response and safety planning with youth and families.
- For this role candidates must have familiarity with hospital systems and discharge procedures.
- Experience working with the Children’s System of Care, Mobile Response & Stabilization Services, and PerformCare through the State of New Jersey’s Department of Children and Families.
- The ability to engage youth and families and build community relationships and partnerships.
- Experience in suicide prevention and terminology, and local resources to provide the youth and families with support.
- Collaborate with other partners within the grant. Understanding of confidentiality and HIPAA regarding medical and identifying information of the youth and families who consent to follow up care.
Education Required:
- Master’s degree in social work, counselling, psychology, or related field.

Pay: $40.00 per hour
