The Program Manager will assist the Program Director by overseeing the administrative operations of the Health Home Program. In this role, the Program Manager will manage both personnel and resources at multiple sites to improve quality, workflows and efficiency, which ultimately supports the goals and objectives of the Health Home Program.

Master's Degree required (MPA, MSW, MA, MPH, MBA or any related field).Clinical license in stated field preferred but not essential for this position. Project or Change Management and Administrative experience in a health care or social service setting.

Strong administrative background with expertise in excel, online databases, change management and performance improvement. Competency in Microsoft products and electronic databases. Must have experience with policy integration in developing and maintaining program operations and managing personnel. Must be organized, an excellent communicator and able to manage multiple and sometimes competing priorities.
