Post-Graduate Fellow in Behavioral Health

CLASSIFICATION/STATUS: Exempt, Full Time, 40 Hours, Temporary (one year appointment)

YOUR ROLE & IMPACT

As part of a dynamic team at Charles River Community Health (CRCH), a healthcare practice providing comprehensive, integrated, and compassionate care to underserved local communities, the Post-Graduate Fellow in Behavioral Health plays a critical role on the primary care team as the bridge between primary care and behavioral health services. The Post-Graduate Fellow in Behavioral Health will provide Case Management and Therapeutic services and consultation during primary care appointments. As part of a passionate team, the Post-Graduate Fellow in Behavioral Health will provide coordinated care to community members who need it the most, and offers the opportunity to assist with program development.

YOUR RESPONSIBILITIES

• Provides psychotherapeutic treatments versed in different modalities, including identification and evaluation of complex risk factors.

• Develops and implements short-term, long-term, and crisis interventions, and provides psychoeducation.

• Triages patient needs and arranges appropriate services for patients either within the clinic's Behavioral Health outpatient team or at affiliated hospitals and clinics.

• Provides consultation to Health Center medical staff, affiliated programs and/or the community as assigned by Behavioral Health leadership.

• will be working to develop outreach materials and participate in trainings/professional development

• Participates in evaluations, records screenings, and interventions in patient medical records according to established documentation standards. Prepares patient reports, correspondence, referrals and other materials for community agencies, schools, legal proceedings and others as needed.

• Uses A-team oriented approach with special emphasis on communication and coordinate of care.

• Follow up with patients and families regarding appointments and referring to other resources. For example Case management, development of resources, and outreach to community providers

YOUR QUALIFICATIONS & COMPETENCIES

• Master’s degree in Clinical Psychology, Counseling Psychology, or Social Work, and completion of all coursework is required.

• Strong interpersonal and communication and analytical skills required.

• Bilingual in English/Spanish or English/Portuguese preferred.

• Experience working with diverse culture and practice with cultural humility preferred.
