POSITION SUMMARY
The Per Diem AIS Nurse is responsible for coordinating and performing infusion therapy in our Ambulatory Infusion Suite as well as conducting patient/caregiver teaching for home infusion services. The Per Diem AIS Nurse facilitates all nursing and operations activities related to the initiation of AIS processes and procedures. The Per Diem AIS Nurse provides clinical and operational support to patients and caregivers for the duration of their AIS infusion therapy and/or infusion teaching, with extensive, ongoing interaction with patients, staff, providers, referral sources, and community agencies. Driving may be required for this position.
TECHNICAL RESPONSIBILITIES
Provides direct patient care, including but not limited to, patient and caregiver education on home infusion therapy in the AIS or at alternate site locations, intravenous line care and access, VAD dressing changes, medication administration via multiple routes (IV, SQ, IM, PO), and laboratory blood draws.- Manages AIS schedule.
- Performs ancillary tasks, including but not limited to, disinfection and cleaning of patient space and equipment and monitoring and replenishing supply inventory.
- Participates in assessment, planning, implementation, and problem-solving for patients who are referred for AIS infusion services.
- Assesses and addresses infusion therapy needs, including but not limited to, vein/venous access device assessment, patient, and caregiver ability to learn and perform home infusion therapy, and selection of the most appropriate infusion method.
- Collaborates with customers (physician, patient/family, health care personnel, etc.) throughout the planning process to establish mutual goals.
- Identifies patient referrals with potential barriers to a safe and effective care plan (e.g., limited or no reimbursement resources, AODA concerns) and collaborates with appropriate stakeholders for evaluation and intervention, as appropriate.
- Monitors patient progress toward admission to service and communicates with referral sources, community agencies, and organization personnel regarding issues which might affect start of care or care plan.
- Recognizes and acts on opportunities to develop individualized and creative care plans for new infusion therapies or unusual patient situations, in collaboration with appropriate organizational leadership.
- Reviews and updates patient information in electronic medical record.
- Provides patients with applicable reimbursement information and education, and connects patients with Patient Reimbursement Coordinator or resources, as needed.
- Forwards all appropriate patient documentation for filing in the patient's clinical record.
- Completes appropriate occurrence reporting.
- Completes all appropriate patient checklists.
- Utilizes quality and safety tools.
- Fully understands and participates in quality and business performance programs, including but not limited to: Monitoring, Fall Risk, and VAD Complications.
- Utilizes acceptable abbreviations according to Company policy.
- Maintains all role-specific competencies.
- Follows “vended” referral process and completes associated tasks.
- Ensures individualized, appropriate, and complete Plan of Care for each patient.
- Performs patient assessments.
- Navigates through outside electronic medical records to obtain relevant patient information and documentation. Abides by internal and external entities’ rules for appropriate access and use.
- Participates in continuing education, as needed.
JOB REQUIREMENTS
Licenses:
Current licensure as a registered nurse in the state of Wisconsin
Education:
BSN Preferred
Experience:
Two years of nursing experience required.- Discharge planning, care coordination, home health, or home infusion therapy experience preferred.
Miscellaneous:
Valid driver’s license and current vehicle insurance required.- Excellent organizational, interpersonal, communication, and customer service skills required.
