Southern Jersey Family Medical Centers, Inc. is seeking a talented Patient Navigators to step into our culturally diverse and fast-paced facility!
Summary: The Patient Navigator functions as a clinical support liaison between patients, providers and care teams members throughout SJFMC and reports to the Director of Nursing. Also coordinates patient-centered care as a liaison between referring physicians, hospital facilities and all other collaborating health care entities. This position will also work in collaboration with any and all applicable departments.
POSITION DUTIES/RESPONSIBILITIES:
Track and monitor provider orders for outpatient services to include laboratory, radiology, diagnostic testing and consultation services.
Create and attach orders for outpatient services requested by specialists.
Ensure appropriate medical records are available at scheduled appointments.
Contact patients and/or specialist as needed for missing reports.
Update demographics pertaining to address and telephone number as needed.
Provide patients with existing copies of lost or misplaced orders upon request.
Facilitate the process for follow up appointments as needed.
Notify clinical staff of patient non-compliance.
Work to improve patient compliance through telephone calls, written correspondence and the patient portal.
Support clinical staff to reduce gaps in care.
Assist with compliance projects upon request.
Routinely review provider patient panels supplied by the various insurance plans to identify patients who have not presented for services.
Assist patients and families in navigating the complex healthcare system.
Attend all mandatory meetings and trainings.
Other duties as assigned or as needed to support organizational goals
Experience and Skills:
Medical Assistants or any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.
Medical terminology required.
Education: Requirement met by License/Certification Requirement
Experience: Two (2) years of directly related work experience in healthcare
Excellent written and verbal communication skills.
Excellent Interpersonal skills.
Excellent customer service skills.
Knowledge of working with Electronic Medical Records (EMR) Systems.
Ability to apply solution based thinking and solve difficult problems with creative solutions.
Excellent organizational skills and the ability to handle high volume and a fast pace.
Ability to work effectively in a diverse environment.
