Family Care Health Centers is seeking a full-time experienced Patient Care Coordinator to work as an integral role in a growing health care team environment. This position is a member of the Health Center's care team and supports the Health Center and its patients to achieve enhanced clinical quality outcomes through patient-centered intradepartmental coordination, The PCC will accomplish this through scheduling patients, performing EHR chart reviews, pre-visit planning and huddling with care teams, coordinating with referral specialists to track patients to ensure test and appointment completing and outreaching to patients. The ideal candidate must be enthusiastic, motivated, hard-working and flexible, be able to multi-task, have a great attitude and thrive in a fast-paced work environment. The Patient Care Coordinator in collaboration with the health care team, is accountable for assisting our patients toward their highest level of health This position reports directly to the Nursing Director. Requirements:

*Bachelor's Degree in business administration, health administration, public health or related field preferred.

*Minimum of two (2) years experience in a a health care setting.

*Experience as Medical Assistant a plus!

*Knowledge of chronic care management preferred.

*Familiar with managed care organizations preferred.

*Excellent verbal and written community skills preferred.
