SUMMARY
The nurse must have open communication and working relationships with staff and family involved in the life of the consumers. The nurse is responsible for developing and monitoring health services for each resident to meet their individual needs. The nurse is responsible for training residential and day program staff in medication administration.
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
ESSENTIAL FUNCTIONS
- Train staff in medication administration using the State of Indiana mandated curriculum.
- Monitor medication administration.
- Complete quarterly nursing assessments for each resident.
- Maintain medical charts to meet applicable regulations. This includes 90-day physician orders, documentation of needed and required evaluations and treatment provided by outside service providers.
- Ensure timely completion of annual physicals, bi-annual eye examinations, annual (or as ordered by dentist) dental examinations.
- Assess residents' health care needs and participate in interdisciplinary decisions as to whether these needs can be met internally or require referral to outside medical/health care professionals.
- Coordinate and schedule medically related appointments with each Group Manager.
- Accompany residents to those appointments which require the nurse's input and others as group home scheduling requires. Also involves home visits to group homes.
- Assist in development of medical and health care procedures for Wabash Center.
- Provide health care information to the Assisted Living Coordinator/QMRP for monthly, quarterly and annual reviews.
- Provide and document annual Mantoux (TB) tests for consumers and staff.
- Provide a minimum of two CPR classes per month. Additional classes may be requires a the request of Wabash Center Administration.
- Assist the Director of Assistant Living Services in ensuring that the Federal ICF/MR standards and the State of Indiana group home regulations are met.
- Provide health care coordination to assigned consumers.
- Assist the Director of Assisted Living Services to ensure that medically related standards for Supported Living are met.
- Ensure the protection of consumer rights in the provision of health services.
- Provide training to consumers in health related areas.
- Ensure all verbal and written communications are presented in a professional manner, reflecting dignity and respect for consumers and other co-workers.

- Must Maintain Current Required Trainings and Certifications in:
- CPR/Basic First Aid
- Medication administration
- Behavioral Intervention Techniques
- Assist the immediate supervisor with various administrative tasks to support department operations as directed; may also be assigned special projects.
- Perform tasks which are supportive in nature to the essential functions of the job, but which may be altered or redesigned depending upon a consumer’s circumstances and/or Wabash Center and regulatory agency policy changes.
- Meet on-going position requirements including training.
- Participate in interdisciplinary team meetings.
- Present in-service training to staff as requested by the Administration.

QUALIFICATIONS
- A current Indiana Nursing License is required.
- Skills in evaluation and assessment of individual's health needs.
- Certified to instruct CPR and Basic First Aid (Or must be obtained within three months of employment).
- Ability to learn and effectively utilize computer software applications to accomplish tasks.
- Ability to read, comprehend, interpret and apply information from general procedures, instructions, data and reports.
- Ability to learn new concepts, which will be provided through departmental training sessions on an ongoing basis.
- Maintain regular attendance and punctuality and accurately maintain attendance records.
- Must provide own transportation, have a valid driver’s license, registration and proof of insurance and acceptable driving record.
- Must have the ability to complete all required trainings and certifications and demonstrate competency of skills taught.

PHYSICAL REQUIREMENT
- Must have the ability to lift/move individuals and/or materials weighing up to 50 lbs. occasionally.
- Must be able to complete fit for duty evaluation based on the individual position where required.
- Must have the ability to stand, walk, bend, stoop, kneel and reach frequently as needed to serve consumers.
THIS POSITION WILL INCLUDE A WORK WEEK OF 5-10 HOURS.
