Position Summary: Conducts screening and assessments to individuals seeking Safe Parking services at various safe parking sites. Partners with outreach teams to provide needed outreach to the unhoused community - linking them with both shelter and housing resources. Works closely with staff to provide input and guidance regarding screening, assessment and subsequent referrals.
Coordinates and initiates services with other public and private agencies providing services to the homeless population. Required Education and Experience: 1.Bachelor’s Degree OR 2.High School Diploma with 2 years of experience providing direct services, preferably with homeless or other vulnerable populations. Preferred Education and Experience: Knowledge of or familiar with community resources in SLO County. License & Certifications: 1.
Obtain CPR Certification. 2. Must have dependable, insured transportation, and a valid California Driver License (mileage to be reimbursed) and acceptable driving record. A DMV printout and proof of insurance will be required.
Essential Functions: 1. Completes check in and check out procedures at various safe parking locations, including but not limited to new participant intakes, screenings, communication of rule violations. 2. Monitor Safe Parking locations for safety, rule violations, and cleanliness and communicate with various internal and external partners to ensure these items are rectified in a timely manner.
3. Assists case managers in outreach, screens and assigns prospective clients for Safe Parking services. Tracks client assignments. 4.
When appropriate, assists Safe Parking participants with assistance in accessing appropriate services such as car repair, job training, etc. 5. Ensures data/client information is entered into Client Track and/or Safe Parking spreadsheet, HMIS and other databases. 6.
Meets with Outreach Case Managers as needed to coordinate services. Works within the Housing Programs team to provide linkage to stable housing. Under supervision of manager assess participant for barriers and provide client centered care plans to help participants achieve stability in housing. 7.
Attends weekly case conference meetings, team meetings, trainings, and public meetings as needed 8. Attends quarterly CEP meetings with community partners to ensure project outcomes. Provide CEP assessments for individuals experiencing homelessness and assist participants in navigation of resources within the CEP. 9.
Provides relevant referrals to agencies and service providers based upon the goals and objectives mutually agreed upon with the client. 10. Maintains a professional relationship with clients and conducts all business related activity according to program policy and standards relevant to the case management field. Supports staff in monitoring client behavior regarding compliance issues and follow-up; provides consistent limits and expectations regarding program policies and goal achievement.
Maintains client confidentiality in accordance with program policies and applicable laws. Responsibilities and Duties: 1. Collects data as needed. 2.
Must be dedicated to the goals and philosophy of the Homeless Services programs and CAPSLO. 3. Must use reasonable precautions in the performance of one’s duties and adhere to all applicable safety rules and practices; and act in such a manner as to assure at all times maximum safety to one’s self, fellow employees, clients and children. 4.
Performs other duties as needed. Knowledge, Skills and Abilities: Knowledge 1. Must have demonstrated knowledge of local human services. 2.
Must be computer literate with basic knowledge of Microsoft Word, Excel and data tracking programs. 3. Must learn Homeless Management Information System (HMIS) within 30 days of hire. Skills 1.
Must have strong verbal and written communication skills. 2. Must have demonstrated organizational skills in former positions. 3.
Must be proficient in required charting and other documentation. Abilities 1. Must have the demonstrated ability to motivate and problem solve in former positions. 2.
Must have the ability to show warmth and appreciation of human growth at all levels and understand the process of gaining trust and credibility with any population. 3. Work Environment: 1. Requires working indoors in a temperature-controlled environment with some exposure to copier and printer chemicals/fumes. 2. The work environment may include some exposure to aerosol transmissible diseases (ATD).
