Responsibilities
The Occupational Therapist develops and implements the home care plan for the occupational rehabilitation of patients under the direction of the attending physician and the appropriate clinical leader.
- Performs initial occupational therapy patient evaluation.
- Establishes and performs occupational therapy treatments according to the physician's plan of treatment and in compliance with state and federal guidelines.
- Provides patient care education to patients and other caregivers.
- Participates in case conferences, summary reports, quarterly record review and annual program review, in-services and special case orientations.
- Maintains productivity standards and availability requirements.
Qualifications
- Bachelor's degree from an accredited school approved by the American Occupational Therapy Association.
- Active registered OT license in the state of Indiana
- One year of OT experience required
- Must obtain and maintain current CPR/BLS Certification (American Heart Association or American Red Cross)
- Valid drivers license, car insurance & reliable transportation
- Passion for excellence in quality of care
