Occupational Therapist, Per Diem Job at Lower Umpqua Hospital District
Lower Umpqua Hospital District is seeking a per diem Occupational Therapist to join our Rehabilitation team.
JOB SUMMARY:
Provide Occupational Therapy services per state standards and practice act as directed by LUH. Duties to be performed in Outpatient, Acute Care, Swing, and SNF settings.
ESSENTIAL DUTIES:
1. Adhere to the Code of Ethics as outlined by the AOTA.
2. Maintain proficiency within the profession by keeping up with continuing education and license.
3. Screens patients for Occupational Therapy needs.
4. Completes initial Occupational Therapy evaluation and performs further assessments and testing as needed per plan of care.
5. Develops plan of care and performs direct patient care using proper modalities and procedures according to treatment plan.
6. Provide regular assessment and revision of the patient’s progress toward goal areas per Medicare guidelines.
7. Maintain records that accurately reflect patient care and progress to include: evaluations, progress reports, and discharge summaries per regulatory standards.
8. Advises, consults, and teaches patient and family members as well as caregivers as indicated.
9. Provide supervision for OTA/COTA’s and rehabilitation aides per state practice act and standards of care.
10. Manage insurance paperwork requirements for patients.
11. Maintain contact with physician regarding occupational therapy plan of care and progress as needed.
12. Attends care conferences and interdisciplinary group team meetings as requested.
13. Participate in Quality Assurance activities as requested
14. Participate in quarterly records review as requested.
15. Comply with hospital policies and procedures.
16. Act as activity program supervisor for hospital swing bed program.
17. Answer phones.
18. Scheduling patients
19. Assist in ordering of supplies.
20. Cleaning of treatment rooms and equipment as needed.
21. Assist other healthcare members – maintaining proper in services for LUH staff.
22. Provide clinical supervision of OT and OTA students and volunteers.
23. Other duties as assigned by the Director of Rehab Services.
QUALIFICATION REQUIREMENTS:
Education and License:
Graduate from an accredited school with degree Occupational Therapy
License in good standing as an Occupational Therapist in the State of Oregon.
Certifications
Basic Life Support (BLS)

1. Must be able to perform physical demands of job duties.
2. Good people skills and be a team player.
3. Good skills in written and oral communications.
4. Must be adaptable to changing work environments.
WORK ENVIRONMENT:
Works indoors in an air-conditioned gym/office environment. Work in Outpatient, hospital, and SNF clinical settings.
PHYSICAL DEMANDS:
Frequent use of computer and phone, and other standard gym and office machines. Must be able to stand, sit, stoop, twist, kneel, reach, push, pull using proper body mechanics and move/lift varying amounts of weight to a maximum of 60 lbs.
