Pay Range: Hourly – Min: $30.00 – Max: $36.49 (based on qualifications and experience)

Job Summary:
The Nurse Home Visitor is responsible for providing comprehensive community health nursing services to women and their families, enrolled in the Nurse Family Partnership, in accord with NFP program goals, objectives and protocols.

Primary Duties & Responsibilities:

  • Assesses physical, emotional, social and environmental needs of women and their
  • families as they relate to health and life course development.
  • Assists women and their families to establish realistic goals and measurable
  • outcomes for promoting their health and life course development.
  • Provides counseling and instruction to assist women and their families in attaining
  • targeted goals, in areas including prenatal and postpartum care, nutrition, parenting, well child care, family planning, and special health problems.
  • Evaluates women and families’ progress toward targeted goals; revises plan of care
  • as appropriate.
  • Develops a working relationship with women and their families that promotes
  • problem solving competence.
  • Promotes involvement of other family members in the pregnancy, birth and early
  • child care.
  • Links women and families with community resources that are relevant to their
  • specific needs.
  • Consults and collaborates with other professionals involved in providing services to women and families on their caseload.
  • Records nursing activities within 24 hours of each visit and turns in paperwork for data entry weekly.
  • Provides feedback for revising protocols related to program implementation.
  • Assumes responsibility for ongoing learning in relation to program implementation.
  • Participates actively on the Nurse-Family Partnership Team.
  • Successfully completes all required trainings.
  • Assists with outreach activities for geographic jurisdiction.
  • Maintains caseload per program protocol.
  • Maintains established frequency of visits with families per program protocols.
  • Maintains average length of home visits of 60 minutes or more.
  • Maintains appropriate proportion of time on Content Domains during different phases of program.
  • Completes required visits per month per program protocol.
  • Meets with Supervisor weekly and completes joint visits per program protocol. (exceptions per supervisor discretion.)
  • Provides timely follow-up on referrals into program-i.e. telephone contact within 48 hours, home visit within 10 working days.
  • Licensed Staff:
  • Performs office nurse duties within scope of licensure as needed
  • Licensed staff will need to be privileged and credentialed as an Other Licensed or Certified Health Care Practitioner (OLCP).
  • A competency check must be completed at the time of the credentialing process.
  • Performs related duties as assigned or required.

Must foster a culture of patient and family centered care, service, quality and safety excellence, while improving patient experience, employee experience and community relations.

Knowledge, skill and ability:
Ability to establish interpersonal relationships with women and families from diverse socioeconomic and cultural backgrounds.
Bilingual skills preferred

Experience:
2+ years professional work experience with childbearing women and children, preferably in a community health setting preferred

Required Education:
Degree in Nursing, Colorado RN Licensure is required.
Bachelor of Science in Nursing (BSN) is required.
