The Cancer Center at the University of Virginia School of Medicine seeks a n avigation outreach specialist. The navigation outreach specialist provides individualized assistance to community members to overcome barriers to accessing and completing cancer screening. The navigation outreach specialist focuses on connecting with community members due for cancer screening and connecting community members to appropriate resources for cancer screening. The navigation outreach specialist serves as a critical link between community members, UVA Health, and other community health care systems, organizations and resources.

Responsibilities:

• Help identify barriers that may be preventing community members from receiving timely, high-quality cancer screening care and identify potential solutions to those barriers.

• Assist patients in informed decisions related to cancer screening

• Collaborate with members of healthcare teams to design and coordinate an evidence-based plan of care. Serve as primary resource for patient to connect them to appropriate resources

• Assists the patient in addressing barriers to care (transportation, language, finances, understanding, etc.)

• Provide patient education by assisting patients in understanding their care and available resources

• Provide extensive understanding of patient care and health care pathways to serve as a conduit between the patient and the health care system.

• Ensure that referrals, procedures, communication, and follow-up occur in a timely fashion through data and problem-solving

• Work with the community, outside agencies and organizations to promote cancer screening awareness and education.

Minimum Requirements

Education : Bachelor's degree required. Additional experience will be required in lieu of a degree.

Experience: Two years of experience required with a Bachelor's degree. Six years of experience required with a High School degree or equivalent.

Salary Range: $53,000-$58,000 with UVA benefits.

Physical Demands

This is primarily a sedentary job involving extensive use of desktop computers. The job does occasionally require traveling some distance to attend meetings, and programs.
