ROLE SUMMARY
The Medical Director of Hospice provides clinical services that reflect the philosophy and mission of end-of-life care: to provide dignity, compassion, and comfort for persons on life’s journey. The care provided will reduce the physical, psychosocial and spiritual suffering and increase the quality of life for Hospice patients and families.
1. Adheres to all Federal, State, hospice & local laws pertaining to the provision of clinical care.
2. Understands and ensures compliance with DOH & CMS policies and procedures for clinical care, safety, infection control, hazardous materials and risk management.
3. Participates in the planning, development & implementation of the Hospice programs.
4. Supports Providers, clinical and administrative staff in providing quality care.
5. Contributes to agency committees, and to staff and community education.
6. Maintains a strong focus in hospice education with both patients and their families.
7. Advances a culture of quality of care and effective teamwork.
8. Proactively identifies opportunities to improve the patient and family experience of care and improve the efficiency and effectiveness of resource use.
9. Works with leadership to enhance the Hospice program's community outreach to Providers, the community, health care facilities, and other applicable organizations in the community.
10. Consults with Clinical ethics committee on End of Life issues.
11. Serves as the ambassador for the Hospice program in our community.
12. Performs Face to Face visits for re-certifications.
13. Reviews the clinical information for the hospice patient and provide written certification that it is anticipated that the life expectancy is six months or less if the illness runs its usual course.
14. Provides full and appropriate documentation of patient care services to support professional billing for these services.
15. Prepares Provider narratives and signatures for Certificates of Terminal Illness and Re-Certification documents.
16. Approves patient admissions.
17. Performs medical admission evaluations to determine care states (general inpatient, routine, or respite care).
18. Conducts daily rounds on GIP patients to include evaluation, clinical management, team communication and documentation.
19. Admission and Discharge Orders for all care center patients.
20. Provides death summaries for deaths in the care center.
21. Communicates with referring Providers on admission, discharge and as needed.
22. Consultation of Routine and Respite Patient when requested by the team.
23. Completes Pharmacy requests for inpatient care
24. On-call responsibilities for GIP admissions to facility or questions/ concerns regarding patient care.
25. Facilitates clarification of patient and family goals of care as needed.

QUALIFICATIONS AND REQUIREMENTS

Education: Doctorate of Medicine (MD) or Osteopathic Medicine (DO) required.
Experience: Minimum of two years’ experience in Hospice required.. Minimum one year experience working as part of a clinical interdisciplinary team required
Licenses/Certificates/Registration:
  • Washington State (DOH) Physician Licensure required.
  • Current state medical license in good standing with medical board required.
  • Board certified (or eligible) in Hospice/Palliative Medicine, or equivalent experience required.
  • Credentialed for full scope of practice at Multicare Yakima Memorial required.
  • American Heart Association BLS or ACLS required.
