PHC provides medical and behavioral health services at its clinic in Palo Alto, and community-based, case management, mental health, and housing services throughout Santa Clara County through our New Directions Program.
PHC’s New Directions program is currently recruiting a Social Work Case Manager to work under our Medical Care Coordination Programs Division. New Directions Social Work Case Managers provide community-based, case management and care coordination services to individuals with complex medical and psychosocial needs in partnership with El Camino Hospital, Santa Clara County Medical Respite Program and Santa Clara Family Health Plan.
Duties Will Include:
- Independently manage assigned caseload.
- Provide in-person, community-based case management services that are strengths-based, client-centered and trauma-informed.
- Complete comprehensive biopsychosocial assessment to evaluate needs and create appropriate care plan.
- In partnership with clients, develop mutually agreed upon care plan goals and steps to be taken toward meeting those goals.
- Responsible for overall care coordination and communication between multidisciplinary providers to ensure provision of services and implementation of care plan.
- Provide crisis intervention for immediate housing needs, medical, mental health and substance use issues.
- Work collaboratively with internal Community Supports providers.
- Make recommendations and referrals to governmental or community-based programs, benefits and services. Directly assist clients with linkage.
- Assist clients to access medical, mental health and substance use treatment providers, including accompaniment to appointments as needed.
- Act as an advocate for clients with social services, community-based agencies, health, and behavioral health providers.
- Work with hospital staff on appropriate discharge plan and follow up post discharge to assess and modify care plan as needed.
- Provide health promotion and self-management training as appropriate.
- Assist clients to function at most independent level possible by addressing ongoing psychosocial needs once clients are stabilized and not in crisis situations.
- Complete documentation, data, and billing requirements in a timely manner per published procedure and timelines.
- Attend and participate in all mandatory meetings and trainings including group, individual and triadic supervision.
- Maintain strict confidentiality of patient information and ensure that patient privacy is maintained.
- Perform other duties as assigned.

Requirements:
- MSW or equivalent required.
- Minimum two years experience working with vulnerable populations such as individuals experiencing homelessness, chronic medical conditions, substance use and/or mental health conditions.
- Demonstrated ability to develop collaborative partnerships with staff, clients, other professionals, and community members, including individuals from diverse cultural and socio-economic backgrounds.
- Valid CA driver’s license, reliable transportation, and legally required insurance.
- Peninsula Healthcare Connection has a vaccination policy that requires employees to prove that they have received the COVID-19 vaccine or have a valid religious or medical reason not to be vaccinated
Qualifications and Skills:
- Knowledge of principles, ethics and best practices of case management, care coordination, social work, and behavioral health services.
- Experience utilizing client-centered, trauma-informed practices to serve vulnerable populations including individuals experiencing homelessness, chronic medical conditions, and/or behavioral health concerns.
- Principles and methods of interviewing, counseling, crisis intervention and assessing psychosocial behaviors.
- Knowledge of human behavior and development.
- Knowledge of community resources and benefit programs.
- Knowledge of the healthcare system in Santa Clara County, particularly safety net providers and services.
- Ability to handle sensitive and confidential materials in a professional manner and in compliance with HIPAA regulations.
- Ability to work on multiple projects, prioritize and meet deadlines.
- Ability to communicate effectively, both orally and in writing.
- Ability to work effectively and efficiently with minimal supervision.
Job Type: Full-time
Pay: From $68,000.00 per year
