Our culturally sensitive team of psychiatrists, psychologists, psychiatric nurse practitioners, case managers and social workers are committed to helping patients facing challenges to navigate through any difficulties, including but not limited to anxiety, work-life balance, relationship issues, trauma, sadness, irritability, or difficulties concentrating.

The Floating Hospital welcomes individuals of all ages, from infants, school age children (5+), youth and adults. We can treat families and will take the time to tailor individual personalized treatment plans and set achievable goals.

The Position:

We are seeking a Life Skills Social Worker who is bilingual in Spanish and will work under the

supervision of the Director of Behavioral Health.

Primary Responsibilities:

Patient Care:

· Assesses patients’ individual situations to determine needs and goals.

· Builds rapport and trust with patients and their families.

· Identifies issues such as abuse, neglect, unemployment, trauma, and poverty to identify services needed.

· Distributes housing packets to DHS patients and reviews the necessary requirements.

· Assists patients in locating viable food sources while promoting healthy eating habits.

· Recommends facilities where clothing can be purchased.

· Develops relationships with legal counsel who will accept referrals for patients in need of legal advice and representation.

· Provides education and guidance around developing financial literacy.

· Provides direction on the importance of being employed and how to maintain a regular job.

· Provides education and support regarding home management, including house cleaning, maintenance, and organizational tips.

· Suggests facilities and organizations that can assist with recognizing developmental delays in children in order to obtain early intervention services and a proper educational pathway.

· Provides information on the educational requirements needed to complete one’s GED.

· Provides information and discusses the importance of filing for financial aid programs that will assist with tuition for advanced educational opportunities.

· Refers patients to the appropriate authorities to obtain appropriate childcare services while employed or while attending school to advance their education.

· Discusses cash assistance and food stamp programs that are available on a temporary basis and how to maximize these benefits.

· Refers patients to behavioral health counseling services as needed.

· Provides necessary follow-up and assistance to ensure goals are being achieved.

Administrative Duties:

· Researches and develops relationships necessary to refer patients to services that are needed to achieve their goals.

· Collaborates with the Medical, Dental, and Behavioral Health teams to identify patients that need services.

· Maintains accurate patient records that will be referred to during follow-up.

· Prepares reports and needed documentation to demonstrate successes and failures

· Other duties as assigned.

Education:

· Bachelor’s Degree in Social Work, Master’s preferred

Experience:

· Familiarity with vulnerable patient populations and the shelter and homeless systems.

· Prior experience working with vulnerable populations preferred.

· Strong knowledge of Microsoft applications, including but not limited to Word, Excel, PowerPoint, and Outlook.

· Must be well organized and have the ability to effectively communicate with patients, staff, and management.

Languages:

  • Fluency in both English and Spanish is a requirement.
