The Pandemic has changed the way we all practice medicine. TeleMedicine is a vital part of closing the gap in healthcare. Chronic Care Management and other patient engagement programs are also important in helping our patients live their fullest life. We have been offering Home Visits since 2019 and have seen the need increase exponentially. In addition, many "otherwise healthy" patients with chronic medical conditions like diabetes, hypertension, dyslipidemia, etc. are in need of one on one accountability with a trusted care provider.

Winnsboro Health Services, a physician owned East Texas practice, is looking for at least 2 LVN's who are comfortable on the phone, want to be part of a great team, and who can encourage patients to take charge of their own health. Typical patients have multiple chronic illnesses and want to improve their level of wellness. We understand that prevention is the key to both a healthier life and a positive outcome. Our goal is to keep the patient engaged in their own healthcare plan and be proactive to address their concerns. Many urban areas offer care management to their high risk patient populations in an effort to decrease acute illness and hospital admissions. We know that patients in rural communities need access as well. TeleMedicine, Care Management and Home Visits fill the healthcare gap experienced by many patients in rural areas. These programs guarantee that all patients get the same (or better) level of care that the big cities have grown accustomed to.

The EMR used is Athenahealth. Training can be done both in office and remotely. You will report directly to our office manager, but you are expected to be able to work independently and efficiently. Our management staff are always available for questions and additional training to help you reach your full potential, both personally and professionally.

Must have experience with the "complicated patient" population, have a wonderful phone temperament, and want to bring compassionate care to every person. Previous home health or hospice experience is a plus!
