Responsibilities:
To act as a resource for individuals, their circle of support and the community services team in creating and maintaining housing services and supports. To ensure provision and payment of supports as outlined in the individual’s plan. To work collaboratively with the fiscal intermediary, community habilitation, finance and accounting teams to ensure appropriate billing and payment of services provided. To act as a liaison/communicate effectively with outside providers, OPWDD, individuals and their families to ensure stability and continuity of housing supports and community habilitation services to those residing in customized residential options. To maintain compliance with applicable OPWDD ADM’s related to ISS, CTS, community habilitation, housing and self-direction and update agency policy as appropriate. To support the mission, vision and values of Heritage Christian Services.
Responsibilities:
- Foster the philosophy of self-direction through active listening and establishment of department outcomes in alignment with the agency’s strategic plan.
- Support individuals, their Circle of Support/Planning Team and Agency employees in person- centered planning and thinking.
- Engage in relationships with individuals, their families/COS and support staff based on mutual respect and compassion.
- Facilitate real life problem solving and conflict resolution as needed with individuals who are self-directing their supports.
- Responsible for oversight/provision of job tasks including:
- Documentation and billing in accordance with regulation and associated OPWDD Administrative Memorandums pertaining to self-directed services.
- Completion of compliance reviews as outlined in the Agency compliance plan.
- Monitor Compliance Inquiries for the Self-Direction team and ensure follow-up is done in a timely manner.
- Payroll and HR functions for self-hire staff and Fiscal Intermediary Coordinators.
- Education and training for self-hire employees.
- Education, training, supervision and leadership development for members of the FI team.
- Monitor finances of the department and adjust as necessary to ensure good financial standing.
- Provide oversight, guidance and support to the Self-Direction Intake Team
- Provide oversight and support to HCS Agency Brokers
- Provide training and support to Independent Brokers working with Heritage Christian Services
- Provide oversight and support to the FI Training Coordinator
- Provide direct supervision to the Associate Directors of Self-Directed Services
- Provide oversight/support to the ISS/CTS transition stipend requests
- Maintain current and comprehensive knowledge of the OPWDD service system, specifically focused on Self-Direction
- Ensure Self-Direction Policy and Procedure is reviewed and updated as needed.
- Act as a Fiscal Intermediary Coordinator for individuals as needed.
- Act as the primary support to individuals in the WNY Customized Residential Options including responsibility to:
- Serve as the primary contact person for the families, other service providers and Community Habilitation management team.
- Under direction of the Associate Director and/or Director of Customized Support, supervises and schedules work of all Community Hab staff to assure appropriate coverage based on individual need.
- Assist in training of staff on an ongoing basis.
- Document service delivery and support colleagues with timely documentation.
- Supervise the facilitation and documentation of each individual’s services and monthly summaries.
- Report all incidents, illness, or any other unusual circumstances to the Associate Director or Director of Customized Support.
- Communicate and ensure resolution to maintenance concerns in the home including general housekeeping and maintenance of the grounds and equipment.
- Train co-workers and role model safe practices in the home inclusive of fire safety, medical emergencies and personal safety.
- Required to have experience in a human service field and at least two years of experience working with people with intellectual/developmental disabilities.
- A QIDP is preferred; however, the minimum of a high school diploma is required.
- Must show excellence in teaching and developing strategies for people with intellectual disabilities.
- Must show evidence of supervisory and administrative skills.
- A valid, New York state driver’s license that is in compliance with Heritage Christian Services, Inc. motor vehicle record standards.
