Gender Health Navigator
The Gender Health Navigator serves as a patient advocate in the care coordination across the healthcare services. This role will help facilitate appointments with primary care providers, specialists, and support services, assist patients in navigating and scheduling gender affirming surgery, and address patient barriers to healthcare. Essential Functions:
- Assist transgender and gender non-binary (TGNB) patients in navigating medical and supportive services including, but not limited to, hormone replacement therapy, gender affirming surgery, name change, and legal services.
- Refer and connect members to services and programming, including gender health care, HIV/STI/HCV testing and treatment, and PrEP.
- Respond to referrals and partner with health educators, medical providers, counselors, case managers, and other staff to provide navigation services.
- Identify health and wellness barriers among members and support individualized and patient-led goal setting.
- Complete care coordination to ensure optimal and desired health outcomes are achieved.
- Conduct retention in care activities, including outreach calls, mailing, and patient-finding.
- Coordinate adherence and retention communication with enrolled clients and patients.
- Participate in outreach efforts to engage new or returning patients.
- Maintains OCH interdepartmental relationships and coordination
Additional Duties
- Interdisciplinary retention in care for individuals living with chronic disease
- Provide other administrative duties as assigned. This includes but is not limited to accurate and timely management of timecards, scheduling requests, attendance, active monitoring of email, intranet, telephone, EMR in baskets, voice mail, and person-to-person communication devices, scheduling, rescheduling, and canceling patient appointments as appropriate, monitoring inventory, organization, cleaning of patient care areas, storage of supplies,
- Other duties as assigned based on program and organizational needs.
Education/Experience Required:
Minimum Qualifications
- High School Diploma or GED
- California Driver's License
- Experience working with/advocating on behalf of members of the Transgender and Non-Binary communities
- 1 year of patient navigation or similar experience in the healthcare setting
- 1 year of experience as a care provider for Black, Indigenous, and other People of Color
Preferred Qualifications
- Experience with EPIC or other electronic medical record systems
- Bilingual in Spanish
