This position works effectively and collaboratively with our community agency and residential partners, fostering good communication and demonstrating reliability.
The In-Home Case Management position conducts comprehensive assessments at the client’s residence, using a laptop computerized system, to determine functional eligibility for Medicaid long-term care services and determination of level of care. This position also develops an individualized service plan in conjunction with the client and/or representative to meet the needs of the client.
This work requires excellent organizational skills to manage a large volume of new referrals and provide diversion activities to prevent hospitalizations or nursing facility placements.
Management would consider hiring a Social Service Specialist 2 in training to a Social Service Specialist 3. The chosen candidate will complete a 12 month in-training plan before joining a permanent unit.
Some of what you'll do:
Conduct functional assessment utilizing the department’s Comprehensive Assessment Reporting and Evaluation (CARE Web) and CARE computerized tool to determine the level of care and eligibility for the Community First Choice (CFC), Community Options Program Entry System (COPES), Medicaid Personal Care (MPC), Residential Support Waiver (RSW) and other programs administered by Home and Community Services for initial in-home services or placement in residential facilities such as Assisted Living Facilities (ALF) and Adult Family Homes (AFH)- The assessment will occur at the client’s home; The CARE Web/CARE assessment will be completed by meeting the quality standards as set forth in the Long-Term Care Manual
- Develop appropriate individualized service plans in conjunction with the client and/or representative to meet the needs of the client. Authorized appropriate services for Medicaid clients utilizing qualified providers if the client chooses to receive their long-term care services in a residential setting or in an in-home setting. Provide skilled case management services to resolve issues, concerns, and diversional activities to prevent hospitalization or nursing facility placements
- Coordinate and provide financial support services including assisting clients to fill out forms and applications for HCS benefits, Non-Grant Medical Assistance (NGMA), verifying Medicaid financial eligibility for CFC, COPES, RSW, MPC, or appropriateness of Fast Track services
- Compute the client’s estimated responsibility when appropriate. Prioritize caseload according to office policy, timeframes, and workload expectations
- Organize, handle, and submit documents per program procedures and office specifications: ensure documents and forms are complete, accurate, and up to date. Participate in unit meetings, staffing with collateral contracts and community partners, mandatory online or in-person training, workshops, webinars, teleconferences, video conferences, and meetings
Who should apply?
Professionals with one year as a Social Service Specialist 2 (formerly Social Worker 2)
OR
A Master's degree in social services, human services, behavior sciences, or an allied field and two years of paid social service experience equivalent to a Social Service Specialist 2
OR
A Bachelor's degree in social services, human services, behavior sciences, or an allied field and three years equivalent to a Social Service Specialist 2
AND
The selected applicant must have one year of paid experience completing comprehensive assessments and developing individualized service plans for the elderly and disabled adult populations including determining the level of care to include care needs related to medical, dementia, and geriatric mental health treatment and interventions
Please note: A two-year Master's degree in one of the above fields that included a practicum will be substituted for one year of paid social service experience.

Communicate clearly and effectively with clients, coworkers, supervisors, community partners and DSHS contractors and vendors- Assess clients in their residence using a portable computer and the agency-designated assessment tool
- Apply knowledge of social casework theory, principles, and practices
- Triage referrals and client care/management
- Link family and clients to agency and community resources
- Resolve complaints between clients and providers
- Identify and evaluate social dynamics affecting client choice and decision making
- Effectively interview clients with or without an interpreter
