Traditions of Mill Creek is searching for a professional, compassionate Director of Wellness (Nursing) to join our team of senior care professionals.

Our community offers a variety of service lines including independent and assisted living and memory care. The residents here are always on the go!

The ideal candidate has 3-5 years of leadership experience in the senior living field, a valid LPN or RN, and a heart for senior citizens. They are expected to maintain the dignity of residents, maintain professionalism with guests and family members, and be a team player. All compliance with state nursing regulations must be maintained, and assistance with employee supervision, evaluation, and counseling are expected.

Responsibilities also Include:

• Working closely with the Wellness Coordinator to ensure proper onboarding of new wellness staff
• Schedule and complete assessments for existing and incoming residents
• Conduct care plan meetings with families, residents, and other loved ones
• Monitor Shift Change Notes and Communication Log.
• Effectively utilize and coordinate services with home health, hospice, and other agencies.
• Ensure physician orders are reviewed, sent to physician, and returned with physician signatures quarterly.
• Monitor to ensure Daily Assignments duties are performed on all shifts.
• Perform quality assurance audits as directed by Executive Director.
• Participates in the Manager on Duty program.
• Must be at least 21 years old.
• Maintain current CPR certification.
• Excellent communication between fellow team members, family members, Vitality leadership, and other visitors of the community
• Must possess excellent organizational skills and multi-tasking abilities.
• Experience with Yardi (or other EMR) is a definite plus!
