We are looking for a Director of Hospice. Must have a minimum of 3 years experience in Hospice. Duties to include: Coordinating referrals from physicians and other sources. Ensuring that hospice services are available, coordinated, and provided in a interdisciplinary manner; Ensure that all services provided under contractual arrangement are supported by written agreement. Ensure that patients/families receive the care and services outlined by the IDT and according to the physician ordered plan of care; Promote communication to ensure optimal care and use of resources for patients, families and other members of the interdisciplinary team; Ensure staffing levels are appropriate for volume of patients, their acuity and the level of services needed. Ensure appropriate on-call coverage. Recruit, select, orient, guide and motivate staff to excel in job performance. Utilize the performance evaluation system for annual evaluation/ corrective action when indicated. Conduct on-site supervisory visits to evaluate staff performance as appropriate; Ensure continuous assessment of each patient’s and family’s needs and verify accurate and timely documentation and updating of assessments, visits, and notifications of attending physicians, and care and services provided; Responsible for fiscal oversight including review and the authorization or denial of claims and invoices from outside vendors and providers. Submission of invoices for payment when necessary; Documentation of all decisions will be maintained by the Administrator; In collaboration with the Quality Manager, coordinates/facilitates short- term inpatient care for pain control, symptom management, or respite services as needed; Take leadership role in development and implementation of policies and procedures of for the hospice program, including the QAPI program, clinical competency, customer satisfaction, and the Infection Control program. Participation in the development of the annual QAPI plan; Ensure medications, medical supplies and appliances, durable medical equipment, clinical bag/equipment and protective personal equipment available to patients and employees; Develop and participate in in-service programs for hospice staff. Ensure competency and performance based evaluations are conducted as outlined in policy. Take responsibility for staying current with home hospice regulations, accreditation requirements. Support and encourage ongoing staff education.; Collect, collate and analyze and report data. Follow up on concerns and/or develop a plan of correction. Experience and requirements: Degree in related field preferred; 3 years experience in hospice; Supervisory experience; Strong team player with a positive attitude; Positive prior work history with great references; Comfort level with electronic documentation; Reliable transportation and valid driver’s license for reasonable local travel.
