Location: Glen Cove NY

Seeking: Full Time, Part Time & On-Call

Hours: days, evenings, and weekends

Areas of Responsibilities:

• Provide support to individuals in the areas of personal care, self-care, communication, behavioral modification, personal interaction, and social relationships
• Transport individuals to social and recreational activities, medical appointments, and other destinations as required
• Help with home organization tasks such as preparing meals, grocery shopping, and cleaning
• Follow individuals daily schedule while implementing all formal goals and protocols
• Collect data such as goals, outings, and progress notes
• Obtain certification for medication administration (AMAP)
• Complete all required trainings and attend staff meetings
• Complete incident reports as necessary

Physical Demands:

• Ability to lift at least 50 pounds or more
• Daily standing, bending and lift
• Must be able to stand for at least eight hours at a time
• Must be able to operate an Agency Vehicle

Qualifications and Requirements:

• Minimum High School Diploma or GED required
• Experience with people with intellectual and developmental disabilities preferred
• Valid NYS Driver License w/minimum 1 year driving experience required
• Flexible to work evening and weekend shifts preferred
