Full Time Position – Normally Monday through Friday during the day, but will have occasional nights, weekends, and on-call responsibilities.
LOCATION: Savage, MN
DESCRIPTION: The job of the Designated Coordinator is responsible for program and direct support staff oversite to ensure compliance with 245D for the Specialized Residential Services (SRS) they are responsible for. Program oversight involves compliance with licensing, quality of programming, and overall satisfaction of the people supported and their support teams. Staff oversight involves hiring, staff training, reviews, scheduling, and corrective action.
The Designated Coordinator is responsible for:
- Knowledgeable about current licensing standards/requirements to ensure compliance with the program they are responsible for.
- Ensures compliance with each person’s Coordinated Service and Support Plan (CSSP)
- Ensures the duties of the direct support staff are fulfilled according to responsibilities in the CSSP and CSSP Addendum.
- Monitor/Evaluate documentation and provide training as needed to ensure consistency in documentation.
- Train the direct support staff and ensure quality of training provided.
- Provide supervision to the direct support staff(s) which includes 90 day and annual reviews, maintaining employee files/notes, and corrective action when necessary.
- Report and document incidents and emergencies, internal reviews, behavioral intervention reporting forms (BIRFs), and other necessary reporting.
- Evaluation of effectiveness of service delivery, methodologies, and progress on the person’ outcomes based on the measurable and observable criteria. Ensure that outcomes are appropriate, and person centered.
- Evaluation and adaptation of Positive Support Strategies that lead to the success of the person supported.
- Conduct mandatory staff meetings on a minimum of a monthly basis.
- Ensure the site has adequate supplies (food, medication, cleaning, etc.)
- Other duties as assigned.
APPLICANT REQUIREMENTS:
Applicants must be able to pass a Department of Human Services background check.
Applicants must be able to organize and work independently.
Applicants must have strong written and oral communications skills.
Applicants must be proficient in Microsoft Office and Google Docs
Applicants must be able to pass skills tests related to physical intervention
Applicants must have familiarity with workforce laws and regulations, unemployment proceedings, etc.
PREFERRED EXPERIENCE/EDUCATION:
Baccalaureate degree in a Psychology, Social Services, or related field AND 5+ years of full-time work experience providing direct care services to persons that struggle with behavioral health concerns
3+ years of management experience in 245D setting
REQUIRED EXPERIENCE/EDUCATION:
This Designated Coordinator must minimally have:
- a baccalaureate degree in psychology, social services, or another related field and 3+ years of full-time work experience providing direct care services to persons struggling with behavioral health concerns.
- an associate degree in a field related to human services, and 5+ years of full-time work experience providing direct care services to persons struggling with behavioral health concerns.
- 8+ years of full-time work experience providing direct care services to persons with behavioral health concerns AND at least 3+ years of management experience.
