The Community Supports Coordinator (CSC) implements and coordinates support services to the highest degree in order that individuals served might be interdependent members and active participants within the community. The CSC focuses efforts on the role of consulting and mediating to facilitate realization of dreams and aspirations. Community Supports Coordinator responsibilities include, but are not limited to:

  • Working direct care as scheduled by the Community Support Supervisor.
  • To assist as request with completing assessments, providing outcome recommendations, complete progress and health reports and attend/participate in person served meetings.
  • Assist in orientation and supervision of new staff.
  • Assist each client with finances.
  • Oversee medical appointments, treatments, etc. of the clients.
  • Assist CSS/LPN with medical paperwork, appointments, ordering supplies and filing the MAR.
  • Other duties as assigned.

This is a part-time position offering approximately 10 hours per week.

This position may require occasional travel to New Ulm Minnesota as well.

A Community Supports Coordinator must have one-year full-time equivalence of direct care with individuals with disabilities or related condition and/or college degree. A high level of sensitivity to the needs of individuals with disabilities. Ability to exercise flexibility and assertiveness, have a positive work ethic, and manage crisis situations effectively and problem solve. Must be able to work without direct supervision.

Must also be able to meet the following qualifications:

  • Valid driver's license and meet qualifiers to be insured by EON, Inc.'s vehicle insurance provider.
  • Must not be disqualified by appropriate criminal background studies.
  • Complete all orientation and training needs as written in policy.
  • Meet the physical requirements as outlined in this position description.
  • Possess good oral and written communication skills and organizational skills.
  • Able to read and write in English.
  • Basic Knowledge of Microsoft Office, Outlook Express, and Internet Explorer.

This is an hourly, non-exempt, position reporting to the Community Supports Supervisor and Health Services Supervisor. Full time and part time positions include every other weekend (Saturday and Sunday). EON, Inc is an employee-owned organization, become an employee owner today!
