Community Living Specialist

The Arrowhead Area Agency on Aging (AAAA), a division of Arrowhead Regional Development Commission (ARDC), seeks a full-time, 32 hours/week, Community Living Specialist to support the Senior LinkAge Line by providing person-centered follow-up with clients over the phone or in-person. Responsible for working with older adults, persons with disabilities and their caregivers to remain in the community or relocate from a long-term care facility back into the community of residence. The specialist will work closely with hospital discharge planning staff, nursing facility staff, Ombudsman and staff of the Senior LinkAge Line Team to ensure the consumer is assisted as seamlessly as possible.

Position Requirements:

· Minimum *bachelor’s or master’s degree from an accredited college or university in nursing, social work, gerontology, or related human services field.

· Two years of professional or internship experience in the social services field, including long-term care, health care provider, social services provider or lead agency setting. Experience in care coordination and case management is essential. *Four years of additional relevant experience may be considered as a substitute for a bachelor’s degree.

· A valid driver’s license.

· If working offsite, employees must meet specific internet criteria including demonstrated highspeed connect ability and the ability to meet privacy and security standards with secure (password protected) internet.

Salary: $23.25/hour.

Location: Employees could be located in any of the seven NE MN Counties (Aitkin, Carlton, Cook, Itasca, Koochiching, Lake, St. Louis) or WI (Douglas County). There are three work options: work in the Duluth office; a hybrid model meaning partial work in the Duluth office and partial work from home; or fully remote.
