Position Overview
Founded in 1971, Centers for New Horizons, Inc. (Centers) is a nonprofit organization that assists children, youth, and families to become self-reliant. With 200+ full-time staff, most of whom live in or near its service communities, Centers provides programs in seven locations throughout the Bronzeville and Riverdale communities of Chicago's Southside that reach over 1,000 individuals daily.

Essential Functions
Conducts outreach activities to key stakeholders and community members for the implementation of Healthy Chicago Equity Zones program for the communities of Fuller Park, Grand Boulevard or Douglas. Builds and strengthens relationships with community organizations to foster partnership development for the health awareness and education of community stakeholders. Participates in community convenings and supports program convenings and activities. Supports the development of key community health partnerships and forums.

Responsibilities
  • Conducts outreach in the community to build strong relationships with community partners and provide health education, materials and support.
  • Plans and implements ongoing health information sessions and presentations.
  • Supports the planning and implementation of convenings tailored to community health priorities.
  • Collects local data on health concerns for reporting.
  • Participates forums for community voice to continuously assess community needs and help create community-led solutions.
  • Disseminates culturally responsive public health messages. Distributes health and safety materials according to established health priorities.
  • Documents all work including convenings attendance, case notes and referrals.
  • As member of a collaborative, supports learning and sharing of best practices within the targeted community.
  • Attends all mandatory program trainings, staff and other team meetings as required.
  • Conduct Neighborhood assessment and network mapping
  • Network with convenings and complete governance documents
  • Adhere to Top 1 priorities and high-level plans
  • Participates in capacity building, shared learning, and health equity ecosystem building activities
  • Adhere to communications plan and support to Public Health messaging
  • Support Policy and Research agendas development and Sustainability plan development.
  • Complete All required contractual, fiscal, and reporting documentation
QUALIFICATIONS
  • Bachelor’s degree in social services or related field preferred, but may be substituted with AA and related experience. Degree or Certification in Public Health related topic highly desirable.
  • A minimum of three years of experience in community outreach and community health.
  • Demonstrable in-depth knowledge of target communities (life, work or play).
  • Must be willing and able to commit to a flexible schedule. Position requires variable evening, weekend and holiday hours.
  • Able to walk or stand for extended periods of time.
Equipment Used :
Employee will generally use the telephone, computer, calculator, copier, overhead projector, paper shredder, paper cutter, and filing systems.
