1. Provides resident care with respect while maintaining resident dignity at all times.

2. Provides resident care in a manner outlined by the resident specific plan of care.

3. Provides resident care using appropriate safety precautions and facility protocols that reduces the risk for resident accidents and hazards.

4. Assists and/or performs activities of daily living (ADLs) according to the resident plan of care: Bathing, Bed Mobility, Dressing & Grooming, Feeding, Toileting, and Transferring

5. Obtain and record resident vital signs per facility protocol and as directed by the licensed nurse.

6. Assists in maintaining a clean and neat environment in the resident rooms:

a. Make beds

b. Keeps bedside tables clean

c. Keeps personal care items in designated areas per facility policy.

7. Accurately document food and fluid intake and output.

8. Clean room or resident area and equipment daily or as often as required to maintain high sanitary standards. Disinfect equipment.

9. Follow facility policies and procedures for all resident care tasks.

10. Reporting resident care and changes in condition to Licensed Nurse supervisors.

11. Follow facility personnel policies and procedures i.e. attendance, cell phones, dress code, assigned breaks, etc.

12. Must be able to care for the cognitively impaired resident using appropriate techniques within regulatory guideline and facility policy and procedures.

13. Be sensitive, patient, and considerate to residents and their families. Always respond in an appropriate and professional way, as the situation requires.

14. Other duties as assigned by Licensed Nursing Supervisors and Administrative Personnel.

REQUIRED EDUCATION AND QUALIFICATIONS:

1. High School Diploma or equivalent.

2. Previous experience in a long-term care setting, geriatrics, or equivalent preferred.

3. Must successfully complete State Registered Nurse Aide training and examination within four months of hire.

4. Must pass administered drug screen and criminal background check.

5. Attend and apply information delivered during ongoing facility training as scheduled.

PHYSICAL DEMANDS:

1. Daily standing and walking during work periods.

2. Daily turning, twisting, stooping, bending, and stretching.

3. Rarely lifting—the policy of PPHC. is that no employee is to manually lift any resident unless use of a mechanical lift is contraindicated by a particular resident’s condition and directed by the Licensed Nurse.

4. Visual acuity—may use corrective device.

5. Hearing acuity—may use corrective device.

6. Finger and hand dexterity.
